The thread may be twisted, but the item is clean with no noticeable scratches or stains. Pearls may have dents or scratches due to their natural material. Pearl thread may be easier to break over time, so replacing the thread is recommended before use.

Cultured pearls. These are Akoya cultured pearls, grown by a pearl oyster with human assistance (not natural pearls).
Treatment. Akoya cultured pearls are routinely bleached and color-toned ("pinked") as standard industry practice.
Natural variation. Minor differences in shape, luster, and surface are natural, not defects.
Care. Keep away from perfume, cosmetics, and chemicals. Wipe gently with a soft cloth after wear and store separately from harder jewelry.
